If you are in need of an accessible vehicle or other adaptive equipment for your child, please submit a request below.  

 

Family must demonstrate that they do not have the resources to afford a wheelchair vehicle on their own. They will also be required to carry car insurance and register the vehicle on their own. We cannot guarantee that the request will be granted. Your submission will go under review. We may request additional verification of income and your child's qualifying medical condition.
